Sacred Heart

The following requirements are to be completed one month BEFORE the baptism date.

INFORMATION NIGHT:

• Baptism Registration Form with a legible copy of the County Birth Certificate. Must be turned in at the INFORMATION NIGHT in order to register.

BAPTISM CLASS:

• The mother, father and each godparent will need to attend the Baptism Class. Baptism classes are held at Sacred Heart every first Thursday of the month from 7:30-9 pm. For more information on the Baptism Class, please see Baptism Renewal Night. 

• If you attend a Baptism Class somewhere other than Sacred Heart, you will need to submit a letter or certificate of completion from the parish where the baptism class is attended, along with the additional required forms.

REQUIRED FORMS:
All "additional" required forms must be submitted together, either in person to parish office or emailed to info@sacredheart.cc as a PDF file, in order to choose a date to have your child baptize.
